Our GAFC program provides personal caregivers and homemaking services for patients who cannot safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLS) including but not limited to:
Epic Nursing Services LLC is accredited for Service Coordination by Commission Accreditation Rehabilitation Facility (CARF) and an approved provider by the Commonwealth of Massachusetts to
provide AFC to eligible MassHealth recipients at least 16 years of age or older who need assistance with activities of daily living provided by a live-in caregiver.
AFC members may stay alone for up to three hours a day. Services include but not limited to:

An individual may qualify for AFC if the individual meets the following criteria:
Caregivers
AFC caregivers are thoroughly pre-screened and qualified as a family member or a nonrelated individual.
Caregivers must be at least 18 years of age or older and determined to be responsible and capable of making sound decisions. In addition to active supervision and assistance, the caregiver will monitor and report any changes in the member's health or well-being to the AFC program staff and submit all required documentation. The member and caregiver reside in an environment that meets qualified setting guidelines.
Through the Adult Foster Care Program, qualified caregivers are paid biweekly a tax-free stipend.

We keep open communication with patient's physicians to advocate for our patients. Using a holistic approach, the RN case manager develops an individualized plan of care collaboratively with the patient, family and multidisciplinary team of professionals to best meet the needs of the patient to ensure achievement of highest quality of life.
