Our GAFC program provides personal caregivers and homemaking services for patients who cannot safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLS) including but not limited to:

• Bathing and/or dressing.
• Feeding and/or toileting.
• Transfer and/or ambulation.
• Meal preparation and/or laundry.
• Shopping and/or errands.
• Medication reminders.
  

• Have Mass Health Standard or a variation of MassHealth to include Senior Whole Health (SWH) Common Care Alliance (CCA) or BMC Healthnet SCO.
• Be at least 22 years of age or older.
• Need daily assistance with one or more personal care task.
  

Epic Nursing Services LLC is accredited for Service Coordination by Commission Accreditation Rehabilitation Facility (CARF) and an approved provider by the Commonwealth of Massachusetts to

provide AFC to eligible MassHealth recipients at least 16 years of age or older who need assistance with activities of daily living provided by a live-in caregiver. 

AFC members may stay alone for up to three hours a day. Services include but not limited to:

• Active supervision and/or assistance to ensure the member's health and well-being.
• Physical assistance with bathing, dressing, toileting, mobility, transfers and/or eating.
• Medication reminders, scheduling, and/or provision of transportation to appointments.

AFC Qualifications

An individual may qualify for AFC if the individual meets the following criteria:

• Has MassHealth for medical insurance.
• Be at least 16 years of age or older.
• Requires assistance to complete Activities of Daily Living (ADLs).
• Has a medical condition or cognitive impairment that requires hands-on assistance or active supervision and coaching to successfully complete at least one of the following activities: bathing or dressing, mobility or transfers, eating or toileting.

Caregivers

AFC caregivers are thoroughly pre-screened and qualified as a family member or a nonrelated individual.

Caregivers must be at least 18 years of age or older and determined to be responsible and capable of making sound decisions. In addition to active supervision and assistance, the caregiver will monitor and report any changes in the member's health or well-being to the AFC program staff and submit all required documentation. The member and caregiver reside in an environment that meets qualified setting guidelines.

Through the Adult Foster Care Program, qualified caregivers are paid biweekly a tax-free stipend.
