 Personal Care Home-making (PCHM)

Our PCHM progam provides personal caregivers and homemaking services for patients who cannot safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLS) including but not limited to:

• bathing and Dressing
• Feeding and toileting
• Transfer and ambulation
• Meal preparation and laundry
• Shopping and errands
• Medication reminders and cueing
  

Case Management

We provide qualified and experienced registered nurses and caseworkers to ensure provision of quality case management services which include but not limited to:

• Transportation coordination
• Social integration and well being
• Wellness nursing visits

We keep open communication with patient's physicians to advocate for our patients. Using a holistic approach, the RN case manager develops an individualized plan of care collaboratively with the patient, family and multidisciplinary team of professionals to best meet the needs of the patient to ensure achievement of highest quality of life.
