Group Adult Foster Care (GAFC)
Epic Nursing Services LLC is accredited for Service Coordination by Commission Accreditation Rehabilitation Facility (CARF) and an approved provider by the Commonwealth of Massachusetts to provide GAFC to eligible MassHealth recipients at least 22 years of age or older who require daily assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to include:
- Bathing and/or dressing.
- Transfers and/or ambulation.
- Meal preparations and/or laundry.
- Light housekeeping and/or grocery shopping.
- Medication reminders and/or cueing.
Adult Foster Care Program (AFC)
Epic Nursing Services LLC is accredited for Service Coordination by CARF and an approved provider by the Commonwealth of Massachusetts to provide AFC to eligible MassHealth recipients at least 16 years of age or older who require daily assistance with ADLs provided by a live-in caregiver.
AFC members may stay alone for up to three hours a day. Services include but not limited to:
- Active supervision and/or assistance to ensure the member's health and well-being.
- Physical assistance with bathing, dressing, toileting, mobility, transfers and/or eating.
- Medication reminders, scheduling, and provision of transportation to appointments.
Qualifications
An individual may qualify for Adult Foster Care if the individual meets the following criteria:
- Has MassHealth for medical insurance.
- Be at least 16 years of age or older.
- Requires assistance to complete ADLs.
- Has a medical condition or cognitive impairment that requires hands-on assistance or active supervision and coaching to successfully complete at least one of the following activities: bathing and/or dressing, mobility and/or transfers, eating and/or toileting.
Caregivers
AFC caregivers are thoroughly pre-screened and qualified as a family member or a nonrelated individual.
Caregivers must be at least 18 years of age or older and determined to be responsible and capable of making sound decisions. In addition to active supervision and assistance, the caregiver will monitor and report any changes in the member's health or well-being to the AFC program staff and submit all required documentation. The member and caregiver reside in an environment that meets qualified setting guidelines.
Through the Adult Foster Care Program, qualified caregivers are paid biweekly a tax-free stipend.
